REGISTER FOr MEALS ON WHEELS in LaSalle, Putnam and Bureau Counties

To register for Meals on Wheels of LaSalle, Putnam, and Bureau Counties, contact the Voluntary Action Center, which provides nutritious meals delivered to your home. The program is available to residents of LaSalle, Putnam, and Bureau Counties who are 60 years or older, their spouses, and disabled individuals. Anyone can refer a resident to the program, including self-referrals, physician’s offices, hospitals, and family members.

Registration Steps:

1. Click here to complete the registration form.
2. You will receive a call from Meals on Wheels staff to schedule a time to complete a nutrition assessment for home delivered meals.
3. With the help of the Meals on Wheels staff, an nutrition assessment will take place in person or over the phone.
