REGISTER TO RECEIVE MEALS ON WHEELS in LaSalle, Putnam and Bureau Counties

Voluntary Action Center provides home delivery of nutritious meals through the Meals on Wheels Program. The program is available to residents of LaSalle, Putnam, and Bureau Counties who are 60 years or older, their spouses, and individuals who are disabled. Anyone can refer a LaSalle, Putnam, and Bureau County resident to the program, including self-referral, physician’s office, hospital, and family members.

Registration Steps:

1. Click here to complete the registration form.
2. You will receive a call from Meals on Wheels staff to schedule a time to complete a nutrition assessment for home delivered meals.
3. With the help of the Meals on Wheels staff, an nutrition assessment will take place in person or over the phone.
